==== Chronic non-cancer pain ==== Guidelines have suggested that the risks of opioids are likely greater than their benefits when used for most non-cancer chronic conditions including headaches, back pain, and fibromyalgia. Thus they should be used cautiously in chronic non-cancer pain. If used, the benefits and harm should be reassessed at least every three months. For treating chronic pain, opioids are an option after other less risky pain relievers have been considered, including paracetamol or nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen. Some types of chronic pain, including the pain caused by fibromyalgia or migraine, are preferentially treated with non-opioid drugs. The efficacy of using opioids to lessen chronic neuropathic pain is uncertain. Opioids are contraindicated as a first-line treatment for headache because they impair alertness, bring risk of dependence, and increase the risk that episodic headaches will become chronic. Opioids can also cause heightened sensitivity to headache pain. When other treatments fail or are unavailable, opioids may be appropriate for treating headache if the patient can be monitored to prevent the development of chronic headache. Starting in the late 20th century, there was an increase in the use of opioids in the management of non-malignant chronic pain. This practice has led to a growing problem with addiction and misuse of opioids, the opioid epidemic.

Binding of pyruvate to the OcDH:NADH:L-arginine complex places the alpha-ketogroup of pyruvate in proximity with the alpha-amino group of L-arginine. The juxtaposition of these groups on the substrates results in the formation of a Schiff base which is subsequently reduced to D-octopine. The priming of the pyruvate site for hydride transfer via a Schiff base through the sequential binding of NADH and L-arginine to OcDH prevents the reduction of pyruvate to lactate.

=== Harmful chemicals === Researchers have detected many PFCs in microwave popcorn bags used as coating materials for oil and moisture resistance. The amount of PFOA in some microwave popcorn bags is determined as high as 300 μg kg−1. Besides PFOA and PFOS, Moral et al. also determined other perfluorocarboxylic acids (PFCAs) in popcorn packaging, including perfluoroheptanoic (PFHpA), perfluorononanoic (PFNA), perfluorodecanoic (PFDA), perfluoroundecanoic (PFUnA), and perfluorododecanoic (PFDoA) acids. Due to the toxicity of PFOA, major U.S. manufacturers volunteered to phase out production of PFOA by the end of 2015. In addition, the use of perfluoroalkyl ethyl-containing food-contact substances are no longer allowed by the U.S Food and Drug Administration (FDA) regulations in January 2016. However, although the production of PFOA and PFOS was reduced, the production of fluorotelomer-based chemicals applied to food contact papers is still increasing. Some compounds, such as polyfluoroalkyl phosphate surfactants (PAPs) or fluorotelomers (FTOH), have been used in some brands of microwave popcorn bags. Those compounds are precursors of PFCAs, and evidence shows that they are more toxic than PFCAs themselves.
